      目的 观察糖尿病神经痛(diabetic neuropathic pain,DNP)大鼠 L4-L6 脊髓背角中小胶质细胞和脑源 性神经营养因子( brain-derived neurotrophic factor,BDNF) 表达的变化及电针对其干预作用,探讨小胶质细胞和 BDNF 是否参与电针镇痛。 方法 SD 大鼠分为正常组(N 组),模型组(M 组),电针组(EA 组),每组 10 只。 EA 组 于第 6 周开始电针双侧足三里和昆仑穴,每次 30 min,隔日 1 次,干预 7 次。 分别检测各组大鼠的基础( base)、4 周 (4 weeks)、6 周( 6 weeks)、8 周( 8 weeks) 空腹血糖( fasting blood glucose,FBG)、热痛阈( thermal paw withdrawal latency,PWL)和机械痛阈(paw withdrawal thresholds,PWT)变化;于 8 weeks 应用免疫荧光法检测大鼠 L4-L6 脊髓背 角小胶质细胞和 BDNF 蛋白的表达。 结果 (1) 6 weeks、8 weeks 时,M 组和 EA 组 FBG 较 N 组显著升高,EA 组 FBG 较 M 组无明显变化;(2)6 weeks、8 weeks 时,M 组 PWL 和 PWT 较 N 组显著下降;8 weeks 时,EA 组 PWL 和 PWT 较 M 组显著升高;6 weeks 时,EA 组 PWL 和 PWT 与 M 组无明显差异;(3) M 组大鼠 L4-L6 脊髓背角 CD11b (小胶质细胞标记物)和 BDNF 平均光密度值较 N 组显著升高,EA 组大鼠 L4-L6 脊髓背角 CD11b(小胶质细胞标记 物)和 BDNF 平均光密度值较 M 组显著下降。 结论 脊髓背角小胶质细胞和 BDNF 可能参与了大鼠糖尿病神经痛 的产生与维持,电针可能通过减少 DNP 大鼠脊髓背角小胶质细胞表达和 BDNF 的表达产生镇痛作用。

      Objective To investigate the underlying mechanisms of the analgesic effect of electroacupuncture (EA), we assessed the effect of EA on microglial activation and the expression of brain-derived neurotrophic factor (BDNF) in the L4-L6 spinal cord in rats with diabetic neuropathic pain (DNP). Methods SD rats were divided into a normal group (N group), a model group (M group), and an EA group (EA group). The rats in the EA group were treated with EA at “Zusanli” (ST36) and “Kunlun” (BL60) starting on week 6, with 30 min per session, once every other day for 7 sessions. Fasting blood glucose ( FBG), thermal paw withdrawal latency ( PWL), and paw withdrawal thresholds (PWT) were measured at baseline, 4 weeks, 6 weeks, and 8 weeks. Immunofluorescence was used to measure the expression of microglia (CD11b) and BDNF in samples from the L4-L6 spinal cord. Results 1) Compared with that in the N group, FBG was significantly increased in the M group and the EA group, and there was no significant difference in FBG between the M group and the EA group at 6 weeks and 8 weeks.(2) Compared with that in the N group, the PWL and PWT were significantly decreased in the M group at 6 weeks and 8 weeks. After EA treatment, compared with that in the M group, the PWL and PWT in the EA group were significantly increased at 8 weeks. there was no significant difference in PWL and PWT between the M group and the EA group at 6 weeks. (3) Compared with that in the N group, the expression of microglia (CD11b) and BDNF in the M group were increased significantly, and compared with that in the M group, the expression of microglia ( CD11b) and BDNF were significantly decreased in the EA group. Conclusions The result suggest that microglia and BDNF in the spinal cord might be involved in the development of DNP. The analgesic effect of EA might be achieved by inhibition of the expression of microglia and BDNF.
